How Your Care Coach Can Help Prep for Neurology Follow-Up Visits

Follow-up visits are a great time to update your care plan, and your Care Coach is here to help you get ready. Before you meet with your coach, jot down a few notes to make the most of your session. After coaching, take a few easy steps before your provider visit to get the support you need.

Review your goals & progress

Your coach will help you review what has changed and what has stayed the same since your last visit.

For your coaching session, bring a short list of two or three improvements, challenges, or changes you have noticed since your last appointment. Keep it simple and use bullet points.

After coaching, pick the main points you want to share with your provider and write a summary. Adding a bit more detail here can help.

Track symptoms & patterns

Your coach can help you notice patterns and find clearer ways to talk about them.

For your coaching session, bring your Neura health tracker. Your coach can look at your logs ahead of time, and the more you track, the easier it is to spot triggers and patterns.

After coaching, write down any new or changing triggers you and your coach discussed. Also, jot down any questions you have about them.

Update your questions

Your coach will help you fine-tune the questions you want to ask at your next appointment.

For your coaching session, bring any important questions you have, even if they’re not fully formed. Your coach can help you organize and reword them if needed.

After coaching, choose your top three or four questions and write them out clearly so they’re ready for your appointment. If you want, you can ask your coach to look them over.

Support a calm, prepared mindset

Your coach can show you simple grounding exercises to help lower stress before your visit.

For your coaching session, think about what has helped you in stressful situations before, and what hasn’t. Your coach will help you use this as you move forward.

After coaching, practice your chosen one or two grounding exercises the day before and just before your appointment.
